This collaborative effort spans five hospitals nationwide, including the Shirley Ryan С (Lead site), Craig Hospital, Baylor Scott and White Health, University of Minnesota, Allina Health, and the Mayo Clinic. Our role in this study involves providing support for blood processing and conducting analyses on circulating Brain-Derived Neurotrophic Factor (BDNF) and genetic polymorphisms. paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>Robot-assisted therapy (RT) is used increasingly as a treatment to improve walking after SCI; the field has seen sustained, rapid growth over the past decade.&nbsp;While RT was expected to provide enhanced benefits over more labor-intensive, less controlled motor learning strategies,&nbsp;so far, it has not demonstrated greater effectiveness than conventional therapy in improving function in patients with a range of neurological conditions. Where some investigators are calling for RT to “trot back to the starting gate” due to its lack of superiority over standard care with respect to functional gains,&nbsp;an alternative scenario that could make RT an attractive supplement to conventional therapy&nbsp;is one in which RT provides similar patient benefits at a lower cost. Indeed, one of the main arguments supporting the continued use and development of RT in rehabilitation has been the assumption that cost savings would be achieved by reducing labor costs.</p> </div> <h3 class="section-header">What is the goal of this project?</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>The goal of this collaborative module was to obtain evidence that informs consumers, clinicians, insurers, and manufacturers about the utilization and cost-effectiveness of robotic exoskeletons in inpatient, outpatient, and community settings.</p> <p>Aims of the project are to:</p> <ol> <li> <p>Describe the physical, psychological, demographic, and socio-economic characteristics, and insurance coverage of patients who receive RT-exo therapy</p> </li> <li> <p>Compare the treatment goals and outcomes of patients who receive RT-exo therapy vs. conventional therapy in terms of functional goals and targeted impairments using performance-based measures (motor strength, endurance, gait characteristics), and patient-reported outcomes stratified by injury level, completeness, and chronicity.</p> </li> <li> <p>Evaluate the cost effectiveness and budget impact of RT-exo therapy vs. conventional therapy to the individual, the health care system, and society.</p> </li> </ol> <p>Findings will inform clinical practice, guide commercial development of rehabilitation robots for human mobility, and assist in planning of clinical trials.&nbsp;</p> </div> <h3 class="section-header">What are some of the results?</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>This project has completed.</p> <p>Some of the findings have been reported in a 2023 paper in the Journal of NeuroEngineering and Rehabilitation, "<a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9867867/"><em>Cost-effectiveness analysis of overground robotic training versus conventional locomotor training in people with spinal cord injury</em></a>."&nbsp;</p> <p>Conventional locomotor training includes treadmill-based training using assistive devices or track-based systems with harnesses and integrated safety systems where therapists manually facilitate stepping. Multiple therapists are often required ensure patient safety and provide support, making this kind of therapy labor intensive for clinicians. Robotically-assisted locomotor therapy includes both treadmill-mounted powered assistive technology as well as stand-alone exoskeletons that can be attached to the patient’s body to facilitate motion. These devices allow the patient to take more steps per session than conventional therapy and lessen the labor needed by therapists.</p> <p>The researchers estimated cost of locomotor therapy and examined data on improvement in quality adjusted life years for 99 patients with SCI undergoing locomotor training at four SCI Model Systems locations: Shirley Ryan С, Craig Hospital in Englewood, Colorado, Shepherd Center in Atlanta and TIRR Memorial Hermann in Houston. Participants were on average 39 years old, 4.8 years post-injury during the study period and 20 percent had a complete SCI.</p> <p>Complete spinal cord injuries result in paralysis below the level of injury. Incomplete spinal cord injuries result in partial damage to the spinal cord and movement depends on the location and severity of the injury.&nbsp;</p> <p>Costs were lower for conventional training ($1758) versus robotic training ($3952), and were lower for patients with incomplete versus complete SCI. Overall, the researchers found that the most cost-effective locomotor training strategy for people with SCI differed based on the completeness of their injury. Robotic training was more cost-effective than conventional training for people with complete SCI while traditional training was more cost-effective for people with incomplete SCI.</p> <p>“Based on current knowledge, it makes sense that full, lower-body robotic exoskeleton-supported physical therapy is more cost effective for patients with complete spinal cord injuries,” said Arun Jayaraman, PhD, Director of the Max Näder Center for Rehabilitation Technologies and Outcomes Research at Shirley Ryan С and a co-author on the paper. “Realizing that these devices are impactful in this more severely impaired population, researchers should focus on modular and soft wearable robots to enhance therapy outcomes in the future.”</p> </div> <div class="paragraph paragraph--mentioned-profile"> paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p><strong>Background:</strong> Engaging patients in therapy is vital in rehabilitation, particularly for patients admitted into inpatient spinal cord injury (SCI) rehabilitation. Literature demonstrates that patients who have higher engagement in therapy tend to have greater functional independence. Certain skills used by occupational or physical therapists may improve patient engagement in therapy (e.g., skills to identify goals that are important to each patient, speaking in a manner that the patient can understand). To ensure that therapists learn and utilize these skills, we have developed a treatment intervention called Enhanced Medical Rehabilitation (EMR). The goal of EMR is to teach therapists a set of behavioral skills so that (1) therapy sessions are patient directed, (2) patients are more active in therapy, and (3) therapists provide more feedback to patients. We previously showed that EMR works to improve outcomes for older adults in nursing facilities. We do not yet know whether EMR works well in inpatient SCI units; thus, we decided to use a well-known adaptation method to customize the EMR program to the inpatient hospital and ensure that it is acceptable to patients with SCI.</p> <p><strong>Objectives:</strong> We propose to adapt the EMR program. We will then test if this adapted program improves therapy engagement and patient outcomes in inpatient SCI units.</p> <p><strong>Methods:</strong> We will randomly assign patients to one of two groups. In one, patients will receive therapy from therapists who are trained and supervised in EMR; in the second, patients will receive therapy from therapists using standard of care (SOC) therapy (i.e., therapists doing what they usually do).</p> <p><strong>Expected Outcomes:</strong> We believe that EMR can be adapted successfully and that the manual and materials can be customized for use in inpatient SCI units. We anticipate that EMR will bring better experiences and outcomes to patients. paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <p>In this Spinal Cord Injury Model Systems (SCIMS) Site Specific Project, our research team will conduct a proof-of-concept project in which we use&nbsp;<a href="https://d3c.isr.umich.edu/experimental-designs/sequential-multiple-assignment-randomized-trials-smarts/"><strong>Sequential Multiple Assignment Randomized Trial (SMART) design</strong></a>&nbsp;to test an intervention to enhance adherence to exercise guidelines in individuals with spinal cord injury (SCI) by using smartphone technology, ecological momentary assessment (EMA) monitoring, and e-coaching.</p><p>EMA is a way to&nbsp;study people's thoughts or behavior in their daily lives by repeatedly collecting data in an individual's normal environment, at or close to the time they carry out that behavior. In this study, we use smartphone technology to gather this data.&nbsp;We believe strategically tailoring intervention components to users’ unique needs and preferences (based on information collected through EMA) will help improve and evaluate rehabilitation outcomes.</p><p>This study is currently recruiting participants for different parts of the study. You can participate in just one part, or both:&nbsp;</p><p>Participants may be asked to be part of a focus group lasting up to 2 hours to discuss app preferences to guide future app development. Participants who have a spinal cord injury may also be asked to complete exercises and other tasks on a customized smartphone app for 2 weeks and then complete a virtual interview and questionnaires. All participants will receive compensation.</p><p><em>To participate, you must meet the following criteria:</em></p><ul><li>Have spinal cord injury diagnosis OR</li><li>Be a caregiver to someone with a spinal cord injury OR&nbsp;</li><li>Be a clinician who regularly treats individuals with spinal cord injury</li><li>Age 18-80 years</li><li>English-speaking</li><li>Willing to use study apps on a personal mobile device</li></ul><p>​​​Participants will complete one in-person visit that may last up to 5 hours. Then they will be provided customized exercises to complete over the next 6 months using either the interactive or more traditional smartphone app (groups are assigned by chance, like a flip of a coin). Up to weekly video conference coaching sessions may be added in the last 3 months. Participation is expected to last 6 months. While some patients experience spontaneous recovery due to preserved neural circuits, many struggle to achieve independent ambulation, and those who do often face persistent deficits. Neilsen Foundation. This project aims to support the mental health of persons living with traumatic spinal cord injury (SCI).</p><p>The research team will conduct focus groups of people with SCI and healthcare providers to identify the psychological needs of the community. With guidance from an Advisory Committee composed of SCI experts, we will select priority topics for six systematic reviews of mental health issues important to adults with SCI. The researchers will share project results with doctors and therapists who are involved in SCI rehabilitation. " class="align-center" width="938" height="625"> </div> <h3 class="section-header">Project Aims and Objectives</h3> <div class="paragraph paragraph--body paragraph--view-mode--default"> <h4 class="field-label visually-hidden">Body</h4> <ul><li><strong>Aim 1: </strong>Identify mental health topics that are important to people with SCI, their care partners, and their healthcare providers.<ul><li>Activities to achieve this aim include six focus groups with persons with SCI and clinicians with backgrounds in mental health and working with persons with SCI will participate in three focus groups. We will also convene an Advisory Committee consisting of persons with SCI, consumer organizations, and healthcare professionals.</li></ul></li><li><strong>Aim 2: </strong>Develop systematic reviews of identified mental health topics<ul><li>Activities to achieve this aim include The systematic reviews will describe the research evidence on effective treatment methods for psychological conditions of persons with SCI. The purpose of systematic reviews in healthcare is to provide a resource to clinicians, researchers, and public health organizations that informs both clinical care and public health decision making.</li></ul></li><li><strong>Aim 3: </strong>Share findings of systematic reviews with primary care and rehabilitation professionals who serve people with SCI<ul><li>Activities to achieve this aim include publishing findings in journals and presenting findings at professional SCI conferences.</li></ul></li><li><strong>Aim 4:</strong> Create and share consumer-friendly resources with actionable guidance about how to access resources that promote mental health and well-being following SCI.<ul><li>Activities to achieve this aim include creating visual abstract infographics that summarize the results of the systematic reviews using colorful graphics to reach persons living with SCI. determine if acute intermittent hypoxia therapy (AIH) combined with transcutaneous spinal cord stimulation (tSCS) during ambulation training improves locomotor function in individuals with spinal cord injury. Use of an orthotic device may help them move more easily by providing them support.</p><p>The&nbsp;<a href="https://www.ottobockus.com/orthotics/solution-overview/c-brace/">C-Brace from Ottobock</a> is one&nbsp;type of next generation micro-processor controlled knee-ankle-foot orthosis (KAFO) that allows users to have more control during the different phases of walking (when the foot first leaves the ground to when it touches back down),&nbsp;offering stumble recovery and reduction in falls. It differs from traditional KAFOs in that it has a micro-processor (computer) that controls the bending and straightening&nbsp;of the knee joint.&nbsp;These features may increase independence in activities of daily living and improve walking safety and stability. If realized, these functions may help people that sustain SCI to increase community mobility and participation.</p><p>The purpose of this study is to evaluate the potential of the C-Brace to improve the functional mobility and quality of life in individuals with lower extremity impairments due to SCI when compared to traditional rehabilitation methods. Participants will be randomized into one of two groups: one that uses the C-Brace, or a traditional care group where participants will receive traditionally recommended orthotics as deemed appropriate by an orthotist. The purpose of the study was to examine the contribution of passive muscle stiffness and active spinal reflex mechanisms to clinical outcomes of spasticity after SCI. It is important that passive and active contributions to increased muscle stiffness are distinguished to make appropriate decisions about anti-spastic treatments and to monitor its effectiveness. To address this question, we combined biomechanical and electrophysiological assessments of ankle plantarflexor muscles bilaterally in individuals with and without chronic SCI.
