Survey on Patient Preferences for Remote Monitoring Using Wearable Health Devices

In collaboration with the Medical College of Wisconsin, the purpose of this study is to develop strategies to include wearable devices into healthcare to remotely monitor health. To participate, we are looking for people who are age 65 years or older and have one of the following conditions: a) Cardiometabolic disease b) Neurological disorder c) Cancer. Summary

Data indicates older adults' healthcare costs average about 3 times greater than those of the typical working adult. In addition to an increased prevalence of chronic disease, functional decline and functional limitations account for 64% of emergency department visits in adults 65 and older. With decreasing time providers have to spend with patients, integrating wearable activity monitors (such as an Apple Watch or Fitbit) into the healthcare setting for older adults to be used to remotely monitor and assess (1) changes to patients functional status, (2) adherence to rehabilitation programs, and (3) early referral and intervention when clinically significant changes are detected. Upon implementation, digital biomarkers can be defined and deployed to reduce risks associated with functional limitations and improve effectiveness of clinical care among the older adult population. However, little is known about how to efficiently and effectively implement wearable devices into an older adult healthcare setting, such as the needs and preferences of the target population for accessing and using these devices.

Therefore, we propose to use a patient-centered approach to determine the needs and preferences of older adults to integrate wearable devices into a healthcare setting. Completion of this study will provide the knowledge necessary to design implementation strategies for integration of wearable devices into healthcare as a means to remotely monitor older adults health in between provider office visits.

Recruitment

Participants must be receiving outpatient or day rehab physical therapy (PT) at Shirley Ryan С��, between the ages of 18-89 years, have a smartphone, and have a diagnosis of one of the following conditions: Parkinson’s disease, Multiple Sclerosis, stroke, post-Covid, lower extremity pain, back pain, or mild TBI/concussion. Study participants are able to use their personal Apple Watch, Fitbit or Garmin devices. Those who wish to participate, but do not own a device are provided with a Garmin device to use. Summary

Exercise and other physical activities are effective physical therapy interventions for people with neurologic disorders, especially when it’s at a high intensity. Unfortunately, there’s currently no easy way for physical therapist to assess how well their patients are following along with the at-home exercise programs they’ve been given.

The popularity of wearable devices like smart watches and fitness trackers have provided an opportunity for physical therapists to gain more objective information about their patient’s everyday exercise and physical activity. Recently, medical facilities have begun using online platforms to secularly share that information with their health care providers. These services, called Remote Therapeutic Monitoring systems, are cloud-based platforms that enable clinicians like Physical Therapists to better guide their patient’s adherence to at home health recommendations.

The purpose of this pilot investigation is to determine the helpfulness, feasibility, and effectiveness of using a Remote Therapeutic Monitoring system to help physical therapists assess their patient’s adherence to their prescribed physical activity and exercise plans and assist clinical care, over the course of 6 months.

Our Remote Therapeutic Monitoring System

We’ve partnered with Datos Health to provide a group of Shirley Ryan С�� Physical Therapists a Therapeutic Monitoring platform for participating patients. Our Datos platform shares the patients’ daily step count, recorded workouts and heart rate during workouts with their treating therapist. This allows the therapists to stay up to date on their patient’s adherence to their prescribed activity goals and reach out to provide guidance. The platform also provides the patients with this activity data relative to their personalized goals. For example, a patient can see how much time during their aerobic exercise they spent in their target heart range zone and how much time was spent below or above it.

The mission of the Rehabilitation Research and Training Center (RRTC) on Employment for People with Physical Disabilities is to research and promote positive employment outcomes for people with physical disabilities.

The Center will investigate how chronic pain affects employment; identify the barriers to and facilitators of job retention; develop strategies that support employment for people with progressive neurological disorders; and create evidence-based decision support for requesting reasonable accommodations under the Americans with Disabilities Act (ADA).

Partners on this RRTC include Northwestern University, the University of Washington, Seattle, the University of Illinois at Chicago and the Great Lakes and Pacific Northwest regional ADA centers.

Summary:

During rehabilitation, many patients and clinicians focus on improving aerobic endurance by gradually increasing exercise intensity for longer amounts of time. By measuring heart rate, we can establish individualized zones of exercise intensity.

This study uses commercially available heart rate monitors and a commercially available software platform to give participants real-time feedback of what zone they’re in during their rehabilitation sessions.

We aim to complete this project across multiple settings: inpatient rehabilitation on the strength + endurance floor and in the DayRehab setting. Our aim in the first phase is to examine how visual heart rate feedback versus no visual heart rate feedback may impact patient and/or clinician behavior, perceptions, and opinions. In later phases, we aim to see if the use of visual feedback of heart rate can help increase the amount of time patients spend in their individualized heart rate zones.

We are also interested in looking at how the Recumbent Seated Step Test, a submaximal exercise test completed on the NuStep, may be helpful to estimate changes in establishing these heart rate zones and track aerobic capacity, and/or establish heart rate zones during rehabilitation.

We will be recruiting people for this study who:

Are undergoing rehabilitation at the site/floor of care
Are between the ages of 18-90 years old
Do not have LVADs, unstable cardiac arrhythmias, or are pregnant
Have physician clearance for participation

This study occurs in conjunction with a patient’s typically scheduled therapies. Participants may be provided with compensation depending on the phase of the study.

Summary

Exercise is important to decrease health risks. Performing at least 150 minutes of aerobic activity per week can lower these risks. There are several questionnaires that clinicians can use to gain insights into a patient’s overall physical activity level as well as screening questions about the amount of exercise they do in a week. However, these surveys have two key problems: (1) they have not been validated in many rehabilitation populations, and (2) they do not ask questions about all types of exercise.

In this study, we are looking to validate exercise and physical activity questionnaires by comparing patients’ response to an activity monitor.

We are actively recruiting people currently seeing an outpatient rehabilitation provider who:

Are between the ages of 18-88 years old
Walk as their primary means of mobility
English is their primary written and spoken language
Do not have a diagnosis of aphasia
Are able to provide informed consent
Are willing to complete a daily activity log and wear an activity monitor for one week

This study requires two visits to the flagship location (355 E Erie Street) to complete questionnaires at the start and end of the study and can be done before or after outpatient appointments. Participants will be compensated for their time.

The Center for Rehabilitation Outcomes Research received 3 years of funding—covering April 2021 through April 2024—from the Craig H. Neilsen Foundation. This project aims to support the mental health of persons living with traumatic spinal cord injury (SCI).

The research team will conduct focus groups of people with SCI and healthcare providers to identify the psychological needs of the community. With guidance from an Advisory Committee composed of SCI experts, we will select priority topics for six systematic reviews of mental health issues important to adults with SCI. The researchers will share project results with doctors and therapists who are involved in SCI rehabilitation. They will also develop and share summaries of the reviews for persons living with SCI.

Project Aims and Objectives

Aim 1: Identify mental health topics that are important to people with SCI, their care partners, and their healthcare providers.
- Activities to achieve this aim include six focus groups with persons with SCI and clinicians with backgrounds in mental health and working with persons with SCI will participate in three focus groups. We will also convene an Advisory Committee consisting of persons with SCI, consumer organizations, and healthcare professionals.
Aim 2: Develop systematic reviews of identified mental health topics
- Activities to achieve this aim include The systematic reviews will describe the research evidence on effective treatment methods for psychological conditions of persons with SCI. The purpose of systematic reviews in healthcare is to provide a resource to clinicians, researchers, and public health organizations that informs both clinical care and public health decision making.
Aim 3: Share findings of systematic reviews with primary care and rehabilitation professionals who serve people with SCI
- Activities to achieve this aim include publishing findings in journals and presenting findings at professional SCI conferences.
Aim 4: Create and share consumer-friendly resources with actionable guidance about how to access resources that promote mental health and well-being following SCI.
- Activities to achieve this aim include creating visual abstract infographics that summarize the results of the systematic reviews using colorful graphics to reach persons living with SCI. The most effective infographics for translating health information to community members offer a lot of information, provide context, support familiarity, facilitate comparisons, and convey accurate meaning even when icons or images are interpreted literally.

Summary

There is strong evidence that increased exercise is associated with slower declines in mobility and quality of life among people with Parkinson’s. However, there are also several barriers to starting and maintaining exercise. Recent studies have shown that clinicians can use digital health technology and behavior change interventions to help people with Parkinson’s to increase their exercise. This project aims to facilitate digital health technology use into physical therapy (PT) practice for people with early Parkinson’s disease.

BARRIERS & FACILITATORS

In the initial phase of study, we conducted interviews with people with Parkinson’s disease, physical therapists, and technology stakeholders. The purpose of this phase was to describe barriers and facilitators to using digital health technology in PT.

Results of the interviews suggest that clinicians should simplify digital health technology in PT to increase use by people with PD. Recommendations for PT include (1) identifying user-friendly features in apps, (2) gathering resources for both clinicians and patients, (3) using individualized approaches, and (4) engaging family, social, and clinical support.

IMPLEMENTATION FIDELITY

We wanted to explore how physical therapists (PTs) can use digital health technology (DHT) and behavior change approaches to help PwP meet exercise goals. We audited the initial session documentation of 30 historic control and 30 participants in the study. All of these sessions occurred in three outpatient rehabilitation clinics with individuals involved in the program for people with early stage Parkinson’s disease.

The PTs had 100% adherence to documenting self-reported baseline exercise for PwP. Implementation aimed to promote the use of DHT and a behavior change approach for exercise adherence in a group of PwP. Implementation strategies included interactive training, facilitation, documentation guidance and clinical resources/education for use with patients.

Trained PTs were more likely to document DHT as a tool for exercise monitoring after training, especially smart phones, fitness apps and wearables. These PTs were also more likely to document detailed information regarding various types of exercise such as aerobic & strength training. See link to the poster for more information below.

Accelerating Physical Therapy Exercise Monitoring through Digital Health Technology for People with Parkinson’s

The purpose of this phase of the study was to explore if physical therapists (PT) can promote digital health technology (DHT) use in people with Parkinson’s (PwP) who were referred to an early intervention, low-dose consultative model of physical therapy. We hypothesized that increased DHT use would be associated with improved self-report of exercise and physical function.

We enrolled 33 PwP over a 15-month period. Patients completed surveys and functional outcome measures at baseline and 6-months. PwP received an initial consultative PT assessment, follow-up DHT support as needed by a research facilitator, and a follow-up research evaluation at 6 months. PwP and PTs completed exit interviews about their experiences.

Recorded frequency and intensity of aerobic and strengthening exercises increased in PT documentation. The number of PwP reporting using DHT ≥5 days/week increased from 11 to 18 and function remained stable, but there was no significant change mean minutes of exercise per week. PwP exit interviews suggest higher adoption of DHT for those who were initially more comfortable or eager to learn technology. PT exit interviews suggest that they value using DHT and scaled DHT recommendations to other patient populations.

These findings suggest that PT education and engagement can positively impact patient-reported use of DHT and facilitate increased exercise in PwP. With this intervention delivery, patients remained functionally stable through 6 months.

Incorporating DHT into physical therapy practice may be valuable to improve exercise adherence for PwP.

National Science Foundation Grant For Improving Human Robot Interactions

Human-like robots, which perform shared tasks with humans in common workspaces, have increased application in home and office services, in rehabilitation and skill training, and in capability enhancement. Digital technologies have enabled these robots with easier and faster human-like audio-visual interactions. However, the lack of physical interaction has posed a significant barrier to human-robot shared tasks, which is essential in applications that require complex physical interactions during the shared tasks. A great number of studies on physical interaction between human pairs have been conducted during the past decade. Several studies showed that dyadic, or paired, physical interaction improves the performance of two individuals working together on shared tasks compared to working alone. Amazingly, results showed that human pairs not only perform better, but also learn new tasks faster. Therefore, understanding how human pairs physically interact can be applied while provide insight in developing human-like robots, which to mimic human behavior when performing shared tasks with other humans or robots.

The vision of this work is to understand the underlying mechanisms of human dyadic physical interaction that lead to improvement in motor performances and learning rates, and to integrate this knowledge in developing new robotic controllers.

Additionally, the infrastructure of the controller and findings will be shared as open source, and educational programs will be developed to support the advancement of the field.

There are many factors that define the physical interaction between dyads, such as the interactive behavior (i.e., collaboration, competition, cooperation), haptic connection (i.e., impedance levels), and skill levels of the dyads (i.e., novice-novice and expert-novice). Therefore, a systematic approach that can quantitatively compare each condition is crucial. To realize the vision of this work, a novel exoskeleton-based dyadic interaction infrastructure will be implemented to study physical dyadic interaction with multiple DOF and multiple contact points by providing virtual connections of varying and controllable impedance between the exoskeleton systems.

This infrastructure will be utilized to reveal comprehensive knowledge of how the task performance and motor learning of peers in dyadic haptic interaction are affected by 1) physical interactive behaviors, 2) impedance of the multi-joint virtual connection, and 3) the skill level of peers. Then, new human-like controllers, namely, co3-robot controllers where the suffix co3 refers to robots endowed with collaborative, competitive, and cooperative human-like interactive motor behaviors, will be synthesized based on a force-impedance adaptation model and a neural network feedback error learning model of interacting peers. Finally, the co3-robot controller will be implemented to a lower limb exoskeleton and will be validated by passing a haptic Turing test, showing that the controller is indistinguishable from a human partner during dyadic physical interactions. As a result of this research, this work has the potential to enhance existing tools and devices with a haptic communication modality, thus supporting joint physical action between humans and robots.

This award reflects NSF's statutory mission and has been deemed worthy of support through evaluation using the Foundation's intellectual merit and broader impacts review criteria.

Gaining Understanding For Better Patient Outcomes

Neurological trauma to the central and peripheral nervous system, such as traumatic brain injury, spinal cord injury, or peripheral nerve injury, can leave a patient with debilitating symptoms and permanently altered functionality.

Rehabilitation outcomes among this subset of patients vary dramatically, even among those with similar injuries. This variance presents an opportunity for therapeutic innovation: if the mechanisms of injury or repair in patients with good long-term outcomes were understood, it might be possible to reproduce them in other patients who currently endure poor outcomes.

The data uncovered in this study will allow us to better understand the individual patient factors affecting the recovery process after neurotrauma and hopefully empower us to predict and influence rehabilitation outcomes.
